COMMUNITY NURSING & POST HOSPITAL CARE
A Nursing 2 You Registered nurse will provide the best possible care for your loved one, attending to your loved one’s medical needs, bringing you peace of mind and make caring for you or your family member easier. Your nurse will manage and ensure that medical care plans are carried out, regularly review, and assess healthcare needs and monitor recovery, inform the Healthcare practitioner of their progress and current condition, monitoring for any changes in condition, and teaching the client how to best manage their disease, symptoms and pain. The registered nurse will also teach the client to use their prescribed medical equipment (Oxygen, blood pressure cuff), support and guide the client to create healthy habits, and promote a healthful lifestyle.
The Registered nurse will provide services including, administering medications, providing wound care and management, monitoring of vital signs, administration of IV Antibiotics, Enteral/tube feeding, drain care, tracheostomy care, Catheter care, Colostomy/Ileostomy care, and managing acute and chronic conditions. Our nurses come to you to provide nursing care and allow you to heal in the comfort of your own home and stay with your loved ones.
PERSONAL CARE & DOMESTIC SERVICES
Home Health Nursing Aids can assist with the day to days tasks while providing companionship. They provide personal care and assistance with meals and other daily tasks so your loved one remains safe in their home. They provide assistance with tasks including personal care, bathing, getting dressed, shaving, oral hygiene, reminders for scheduled medications, health care escorts, accompanying to and from medical appointments, going grocery shopping, companionship (Going for walks, doing crossword puzzles, etc), and help around the home with things related to their care to make sure they are safe. Nursing 2 You offers much needed support, compassion and respectful care.
AGED CARE & PALLIATIVE CARE SERVICES
Our Nurses help manage chronic conditions such as cancer, COPD (Chronic obstructive Pulmonary disease), Diabetes Mellitus, CHF (Congestive heart failure), Parkinson’s disease, Dementia Alzheimer’s disease, and stroke. We provide support and care at home so that your loved one can continue to live at home within their community as they age or, as disease progresses, and provide support to caregivers.
